I use uptime-kuma for this. It’s already implemented in the cp, give it a go, it’s fairly easy to use.

We have implemented a script which actively monitors all supported installed apps, if any app goes down then it will send a notification on your discord and restart it, automatically.
It also saves the alert in a log file for future troubleshooting.
